1) Trade License, (First and current)  2) Pan Card,  3) Voter Identity Card,  4) Proof of Owners Residential Address,  5) Bank Statement with Account Number showing last six months transaction,  6) Rent Bill, with copy of deed/agreement if hired/Municipal Tax Bill if owned,  7) Labour Licence issued by Labour Commissioner Govt. of West Bengal (In case of contractor), 8) Attendance Register, Salary Register for the month of No. of employees 10/20,  9) Partnership Deed (In case of Partnership firm),  10) Recent work orders (Minimum Two),  11) Memorandum and article of Association (In case of Ltd. Company), 12) Form 32 of Companies Act, in case there is change in Directorship,13) Shop & Establishment License for Shop & Security, 14) Factory License under section 2(m) (i) or 2(m) (ii) and proof of power connection for manufacturing process as per section-2(k) of the Factory Act in case of power using factory.15) List of worker employed,their Declaration forms alongwith two copies of recent postcard size attested photographs and Bank Draft towards payment of advance contributions for six months in respect of such workers from the date they become coverable (Column 15 for Security Agencies/Labour supplying Contractors/Placement Agencies etc.)
